Introduction

The purpose of this website is to give young people with cerebral palsy and other physical disabilities information to help them get ready for their move to the adult healthcare system. This website will also be useful for parents. However, we have created an information booklet specifically for parents and guardians. You can find this on the Resources page.

This information and resources were informed by findings from a research project examining transition among young people with cerebral palsy in Ireland. You can read about the project and the findings on the project website.

While reading this information remember that everyone has a different health and life journey. So having one way of preparing for transfer to adult services is not the best way for everyone. Transition will look different for each young person and family depending on their abilities, needs and the options available.

While this information is primarily focused on the transition from child to adult health services, it’s important to keep in mind that the transition into adulthood involves transitions in many other areas of life. These may include education, living options, relationships, and employment. Some of these topics are covered on this website but not in detail. We provide links to other resources where you can find more information about these topics.

You may find it helpful to discuss these aspects of transitioning to adulthood with your parents, health professionals, educators, or others who support you.
