This website was originally created to support young people with cerebral palsy as they move from children’s to adults’ health services. It was subsequently updated to include information relevant to other young people with disabilities.
You can download an information booklet about transition and other resources at the link below. This website was developed with funding from the Central Remedial Clinic and Health Research Board.
